Electrophysiology Model for a Human Heart with Ischemic Scar and Realistic Purkinje Network

چکیده
We present a Purkinje-myocardium electrophysiology model that includes the scar plus a detailed Purkinje network, and compare simulated activation times to those obtained by electro-anatomical mapping in vivo. The results illustrate the importance of using sufficiently dense Purkinje networks in patient-specific studies.

Generating Purkinje networks in the human heart.
The Purkinje network is an integral part of the excitation system in the human heart. Yet, to date, there is no in vivo imaging technique to accurately reconstruct its geometry and structure. Computational modeling of the Purkinje network is increasingly recognized as an alternative strategy to visualize, simulate, and understand the role of the Purkinje system. However, most computational mode...
متن کاملIschemic Ventricular Tachycardia Presenting as a Narrow Complex Tachycardia
This report describes a patient presenting with a narrow complex tachycardia in the context of prior myocardial infarction and impaired ventricular function. Electrophysiological studies confirmed ventricular tachycardia and activation and entrainment mapping demonstrated a critical isthmus within an area of scar involving the His-Purkinje system accounting for the narrow QRS morphology. This v...
متن کاملA coupled 3D-1D numerical monodomain solver for cardiac electrical activation in the myocardium with detailed Purkinje network
We present a model for the electrophysiology in the heart to handle the electrical propagation through the Purkinje system and in the myocardium, with two-way coupling at the Purkinje-muscle junctions. In both the subproblems the monodomain model is considered, whereas at the junctions a resistor element is included that induces an orthodromic propagation delay from the Purkinje network towards...
متن کاملConversion to Purkinje-Related Monomorphic Ventricular Tachycardia After Ablation of Ventricular Fibrillation in Ischemic Heart Disease.
BACKGROUND Catheter ablation is an effective therapy for ventricular fibrillation (VF) arising from the Purkinje system in ischemic heart disease. However, some patients experience newly emergent monomorphic ventricular tachycardia (VT) after the ablation of VF. We evaluated the prevalence and mechanism of monomorphic VT after VF ablation. METHODS AND RESULTS Twenty-one consecutive patients w...

This study uses an anatomically detailed mesh of the rabbit heart with a coupled electromechanical model in order to determine optimal left ventricular lead placement in Cardiac Resynchronization Therapy. The mesh and models were created in Continuity 6, developed by the Cardiac Mechanics Research Group in the University of California, San Diego. Continuity 6 is a finite-element modeling softwa...
